This form is not just for athletes. We recognize that our younger students will not be able to sign the form, and a parent signature is sufficient. This form MUST BE RETURNED TO OUR OFFICE ON OR BEFORE THE FIRST DAY OF SCHOOL. If we do not have a signed form, your child will not be allowed to participate in gym class.
